Pathwell Health is seeking a Speech Therapist for home health to assess, plan, and treat patients with speech, language, swallowing, and cognitive needs. The role involves evaluating function, developing plans of care, and coordinating with the care team to improve patient well-being.
The therapist will document progress, communicate with nurses and physicians, and provide family training. CT licensure and strong clinical skills are essential for success in this position.
The Speech Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient’s well-being, and evaluates the patient’s progress.
