Concierge Home Care, Sarasota, FL, is seeking a Speech-Language Pathologist to provide skilled services in patients' homes. You will assess, treat, and monitor speech, language, cognition, and swallowing, delivering patient-centered care with autonomy.

Qualifications

• Active Florida Speech-Language Pathologist license in good standing.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.

Responsibilities

• Conduct comprehensive speech, language, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on communication, cognition, and swallowing.
• Complete Medicare OASIS documentation as required.
• Document visits efficiently using AI-assisted tools.

Your Role as a Home Health Speech-Language Pathologist

As a Home Health SLP , you will provide skilled speech therapy services in the comfort of patients’ homes while helping them improve communication, cognition, and swallowing function.

Responsibilities include:

• Conduct comprehensive speech, language, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on communication, cognition, and swallowing disorders
• Educate patients and caregivers on techniques to improve functional outcomesCollaborate with physicians and interdisciplinary care teams
• Complete Medicare OASIS documentation as required
• Document visits efficiently using AI-assisted tools
