Job summary

Concierge Home Care seeks a licensed Speech-Language Pathologist to provide in-home therapy in Sebring, FL. You’ll evaluate, treat, and educate patients with communication, cognition, and swallowing disorders, while coordinating with physicians and care teams.

You’ll enjoy flexible scheduling, PRN/1099 options, technology-assisted documentation, and a supportive environment that values work-life balance. AHCA Level 2 background screening may be required.

Qualifications

• Active Florida SLP license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.

Responsibilities

• Conduct comprehensive speech, language, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy in patients' homes focusing on communication, cognition, and swallowing.
• Educate patients and caregivers on techniques to improve outcomes.
• Document visits and complete Medicare OASIS as required.

Your Role as a Home Health Speech-Language Pathologist

As a Home Health SLP , you will provide skilled speech therapy services in the comfort of patients’ homes while helping them improve communication, cognition, and swallowing function.

Responsibilities include:

• Conduct comprehensive speech, language, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on communication, cognition, and swallowing disorders
• Educate patients and caregivers on techniques to improve functional outcomes
• Collaborate with physicians and interdisciplinary care teams
• Complete Medicare OASIS documentation as required
• Document visits efficiently using AI-assisted tools
