Job Title:
Speech Therapist
Job Purpose or Summary:
Provide comprehensive evaluation for Speech Language diagnoses for patients/residents with communication and/or swallowing deficits. Provide written reports to contracted facilities or agency and patient/resident's physician. Establish and carry out treatment programs, which will meet the patient/resident's functional communicative/swallow needs. Interact regularly with other members of the rehabilitation team and medical staff on patient/resident's treatment plan, progress, and prognosis.
Responsibilities:
  • Provides differential diagnosis for patients/residents with speech, language, swallowing disorders.
  • Provides written reports to both contracted facility or agency and patient/resident's physician.
  • Establishes and carries out treatment programs on the basis of the diagnostics information available which will meet the patient’s functional communicative needs.
  • Educational instruction to the patient/resident, family, and appropriate restorative personnel regarding the communication disorder to facilitate recovery.
  • Interacts regularly with other members of the rehabilitation team and nursing staff on the patient's treatment plan, progress, and prognosis.
  • Develops appropriate home and community programming to maintain and enhance the performance of the patient in their environment.
  • Terminates service when benefit has been achieved.
  • Maintains good rapport with all significant administrative and restorative nursing personnel to facilitate good communication and coordination of treatment program.
  • Assures that all required written documentation is completed and appropriately placed on a timely basis.
  • Complies with established agency standards and evaluation compliance.
  • Submits logs and all billing information on a timely basis.
  • Reports any problem areas within each facility to the appropriate supervisor immediately.
  • Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines.
