Job summary

Uintah Basin Rehabilitation and Senior Villa in Roosevelt, UT is seeking a Speech Therapist to join our growing team. The role is on-call but full-time in terms of benefits eligibility, with services provided under physician orders and supervision of the Clinical Manager, serving residents at their place of residence.

Applicants must hold a master’s or doctoral degree in Speech Language Pathology and be licensed in the state. Two years of experience is preferred; competitive benefits are offered.

Qualifications

• Must hold a state licensure as a Speech Language Pathologist.
• Two years of experience in speech-language pathology preferred.
• Currently licensed in the state where practicing.

Responsibilities

• Implements speech therapy services as prescribed by physician orders.
• Evaluates patients and documents progress.
• Plans and provides full range of Speech Language Pathology services.
• Supervises and directs assigned personnel as needed.
• Interacts with physicians regarding treatment adjustments.
• Maintains patient records and progress reports.
• Educates patients and families on home programs.
• Participates in patient care planning with the care team.

**Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.**

Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist

Responsibilities

• Improves or maximizes the communication of the patient.
• Returns the individual to optimum and productive living within the patient’s capabilities.
• Periodically participates with all other home care personnel in patient care planning.
• Provides full range Speech Language Pathology Services as ordered by physician
• Directs and supervises personnel, as assigned.
• Takes initial history and makes initial evaluation.
• Performs all skilled procedures.
• Consults with physicians regarding change of treatment.
• Writes reports to physicians regarding patient’s progress.
• Instructs patients and family members in home programs.

Requirements

• A master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or
• Must have successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national examination approved by the Secretary.
• Currently licensed in the state(s) in which practicing.
• Two (2) years’ experience, preferred.
