Job summary

Granite Peak Home Health is seeking a dedicated Home Health Speech Therapist to provide in-home speech therapy services in Miles City, Montana. This per diem role requires compassionate, patient-centered care and the ability to develop individualized treatment plans for diverse patients.

The ideal candidate will have a Master’s in Speech-Language Pathology, state licensure, CCC-SLP (preferred), and at least 2 years of home health experience, with strong communication and driving flexibility to

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current state licensure as a Speech-Language Pathologist.
• CCC-SLP certification preferred.
• Minimum of 2 years of experience in speech therapy, preferably in a home health setting.
• Strong clinical skills in assessment and treatment of speech, language, and swallowing disorders.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive assessments of patients’ speech, language, and swallowing abilities.
• Develop and implement individualized treatment plans based on assessment findings.
• Provide direct therapy services to patients, utilizing evidence-based practices.
• Educate patients and their families on strategies to improve communication and swallowing.
• Monitor and document patients’ progress, adjusting treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely patient records in compliance with company policies and regulatory requirements.
• Participate in team meetings and continuing education opportunities to stay current with best practices.
