Job summary

Granite Peak Home Health in Miles City, MT, is seeking a licensed Physical Therapist to join our team. This role covers Miles City and surrounding areas, with a Monday–Friday schedule and weekends as needed.

The PT will provide physician-prescribed therapy and collaborate with other home health staff to optimize patient outcomes. The ideal candidate will hold an MT license, have knowledge of HIPAA and healthcare regulations, and bring experience in home health.

Qualifications

• Graduate from a physical therapist curriculum approved educational institution.
• Currently licensed in the state of Montana
• 1 year home health experience preferred
• BLS/CPR required for this position
• Knowledge of HIPAA, patient privacy and healthcare regulations

Responsibilities

• Adheres to agency policies and procedures.
• Provides physician-prescribed physical therapy.
• Improves or minimizes patient disabilities.
• Returns patient to optimum function within capabilities.
• Participates in care planning with home health personnel.
• Directs and supervises staff as needed.
• Manages initial assessment, plan of care and updates every 30 days.
• Performs skilled procedures per physician orders.
• Consults with physicians on treatment changes.
• Writes progress reports to physicians.

Qualifications:

• Graduate from a physical therapist curriculum approved educational institution
• Currently licensed in the state of Montana
• 1 year home health experience, preferred
• BLS/CPR required for this position
• Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements
• Knowledge of the local healthcare market and community outreach strategies
• Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR)
• Clean driver’s license with a clean driving record

Principal Responsibilities:

• Understands and adheres to established policies and procedures
• Provides physician-prescribed physical therapy
• Improves or minimizes residual physical disabilities of the patient
• Returns the individual to optimum and productive level within the patient's capabilities
• Participates with all other home care personnel in patient care planning
• Directs and supervises personnel as required
• Responsible for initial assessment, plan of care, maintenance program development, and modifications and reassessments every 30 days
• Performs all skilled procedures as ordered by the physician
• Consult with physicians regarding changes in treatment
• Writes reports to physicians regarding patient's progress
• Instruct patients and family/significant others in-home programs and activities of daily living
• Participates in in-service programs and presents in-service programs as assigned
• Participates in Quality Assurance and Performance Improvement activities as assigned
• Attends all patient care conferences as scheduled
• Prepares clinical and progress notes
• Assists physician with evaluating level of function
• Helps develop the plan of care and revise it as necessary
• Consult with family and Agency personnel
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition under Agency defined time frames
