Job Responsibilities:

• Conduct assessments of residents' speech, language, cognitive, and swallowing abilities.
• Develop individualized care plans for communication and swallowing difficulties.
• Ensure accurate collection of Section GG data for various assessments.
• Administer speech therapy interventions and therapies.
• Maintain detailed documentation of evaluations, treatment plans, and progress notes in EMRs.
• Educate residents, families, and staff on mobility and therapy exercises; conduct remote check-ins as needed.
• Collaborate with interdisciplinary team members for holistic resident care.

Qualifications:

• State licensure as a Speech-Language Pathologist
• CPR/BLS certification
