Job summary

LiveWell Homecare Agency, LLC seeks a PRN Speech-Language Pathologist to provide acute care coverage focusing on communication, cognitive, and swallowing disorders. You will evaluate patients, develop and implement personalized treatment plans, and collaborate with physicians to optimize outcomes.

In this role, you will conduct assessments, implement therapies, document progress, educate patients and families, and ensure regulatory compliance.

Qualifications

• Master's degree in Speech Pathology from an accredited program.
• Licensure as a Speech-Language Pathologist and BLS certification.
• Effective communication, customer service orientation, teamwork, and basic computer skills.

Responsibilities

• Conduct patient assessments and establish care plans aligned with patient and family goals.
• Implement and adjust speech, language, voice, cognition, and swallowing therapies.
• Document evaluations, treatment sessions, discharge instructions, and referrals per departmental standards.
• Provide patient and family education, discharge planning, and follow-up care.
• Ensure compliance with regulatory standards and maintain confidentiality.
• Participate in team meetings, professional development, and mandatory trainings.
• Adhere to ethical practices, promote diversity, and foster a respectful environment.
