Job summary

LiveWell Homecare Agency, LLC is actively seeking a Speech Pathologist to evaluate, assess, and develop personalized treatment plans for patients with speech and communication challenges. You will identify issues, modify therapy as needed, and track patient progress in a supportive rehabilitation environment.

A current licensure or certification per state regulations is required, with CCC certification preferred and CPR certification to be obtained within 30 days if not already held.

Qualifications

• Current licensure or certification per state regulations.
• SLP Certification of Clinical Competence (CCC) is required or strongly preferred.
• CPR certification obtained within 30 days if not already held.
• Master's degree preferred, or Bachelor's degree with relevant experience.

Responsibilities

• Evaluate, assess, and develop personalized treatment plans for patients with speech and communication challenges.
• Identify issues and modify therapy as needed.
• Track patient progress and document outcomes.
• Collaborate with rehabilitation team to support patient recovery.
