Job summary

Foundation Health Partners is seeking a Speech-Language Pathologist to provide screening, evaluation, and rehabilitation for patients with speech, language, cognitive-communication, voice, and swallowing disorders in Fairbanks, Alaska. This role involves VFSS and fluency, developing specialized treatment plans, and documenting care for diverse ages.

Qualifications

• Masters degree in Speech Language Pathology from an accredited program.
• Current licensure in the state of practice.
• Certificate of Clinical Competence (CCC) or documented acceptance in Clinical Fellowship Year (CFY).
• BLS certification required.

Responsibilities

• Evaluate patients’ functional abilities and limitations to determine intervention goals.
• Develop a plan of care and treatment goals with physician/refer source.
• Implement individualized treatment plans across communication, cognition, voice, swallowing, and fluency disorders.
• Educate patients and families; provide home programs and instructions.
• Document patient treatment and participate in staff training as needed.
