Summary

The Speech Language Pathologist (SLP) provides comprehensive skilled care as ordered by the attending physician within specified geographical territory. The SLP educates patients and family members and ensures patient safety. The SLP completes accurate documentation and remains compliant with all legal rules and regulations.

Schedule & Territory

Schedule: 3 days a week Monday-Friday. Territory: Millersport, Adelphi, Amanda, Baltimore, Bremen, Carroll, Reflections-Lancaster, Lancaster, Laurelville, Pleasantville, Rushville, Stoutsville, Sugar Grove, Tarlton, Thurston, Kingston, Thornville, Ashville, Bloomingburg, Circleville, Commercial Pt., Derby, Harrisburg, Jeffersonville, New Holland, Orient, Washington Ct. House, Williamsport, Reynoldsburg, Brice, Canal Winchester, Groveport, Lithopolis, Lockbourne, Pickerington, Columbus, E. Cen Columbus, German Village, S Columbus, Columbus, Columbus - NE Central, Whitehall, Rickenbacker, Columbus (Airport), Linwood, Edgewater Park.

Qualifications & Attributes
  • Current Speech‑Language Pathologist (SLP) license in the state of employment.
  • Minimum of 1 year of experience as an SLP is preferred.
  • Home care experience desired.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians and co‑workers.
  • Knowledgeable on speech therapy best practices.
  • Ability to make appropriate speech therapy judgments.
  • Ability to identify situations and address with best solution.
  • Detail‑oriented and observant.
  • Disciplined work ethic; prioritize and be timely.
  • Valid driver’s license and auto insurance with your name as a listed driver.
Major Areas of Responsibility
  • Provide skilled speech therapy as ordered by the attending physician.
  • Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Develop an individualized speech therapy plan of care and adjust as needs change.
  • Observe, record, and report patient reactions, changes, and progress to attending physician and agency staff.
  • Inform health care staff through certain phases of speech therapy for patient use.
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and other speech‑therapy‑related functions.
  • Guide the patient in therapeutic, creative and self‑care activities to improve functioning.
  • Promote continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel and instruct patient and family on disease/injury/disability and management in all phases of speech therapy.
  • Incorporate patient and family in development of plan of care.
  • Instruct patient and family members on proper use of procedures and equipment.
  • Teach preventative measures to decrease re‑hospitalizations.
  • Listen to patient and family members.
  • Complete clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
  • Document all aspects of treatment, assessments, and patient education.
  • Complete chart audits as requested.
  • Communicate with scheduler about changes as needed.
  • Participate in on‑call requirements and case conferences.
  • Attend in‑service trainings and mandatory agency meetings.
  • Participate in education programs and in‑services mandated by state law.
  • Participate in orientation/training of new employees.
  • Maintain professional appearance representative of company.
  • Understand and practice agency policies and procedures.
Health Qualifications
  • Meet all physical position requirements.
  • Provide negative tuberculin skin test or x‑ray; annual TB test required.
  • Free of physical/medical conditions limiting job functions.
  • Physical demands include reading, speaking, writing, computer usage, sitting, hearing, vision, walking, bending, standing, driving, lifting up to 50 lbs, stooping, stretching, hand dexterity, etc.
  • Travel is required.
