Job summary

Enhance Rehabilitation in Illinois is seeking a qualified Speech Language Pathologist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care developed after evaluation.

You will communicate effectively with patients, families and the treatment team, and you will have experience with standardized assessments, swallowing evaluations, developing plans and monitoring progress.

Qualifications

• Provide patient care in SNF setting with appropriate clinical knowledge.
• Strong written, oral and interpersonal communication with patients, families and the treatment team.
• Experience with standardized speech/language assessments, swallowing evaluations, developing treatment plans and progress monitoring.

Responsibilities

• Evaluate patients and treat them following an individualized plan of care after evaluation.
• Demonstrate the knowledge to provide patient care in a Skilled Nursing Facility (SNF).
• Collaborate with the treatment team to gather and exchange patient condition information.
