Job summary

Evolve Therapy Services is a therapy management organization seeking a compassionate Speech-Language Pathologist to join our in-house team at a Skilled Nursing Facility in Burlington, WI.

As a licensed SLP with CCC, you will evaluate residents, develop treatment plans, document in NetHealth EMR, and collaborate with staff to ensure high-quality care. Full-time with sign-on bonus or PRN options are available for eligible candidates.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC and active state SLP license required.
• Ongoing CEU to maintain licensure.

Responsibilities

• Provide quality speech therapy services following standards and productivity guidelines.
• Evaluate and collect data for treatment planning.
• Develop treatment plans with goals.
• Instruct patient, family, and staff for carryover of therapeutic remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time (Sign On Bonus) or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
