Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our care team. The role provides direct patient care, conducts assessments, and develops individualized treatment plans for rehabilitation patients.

The SLP will collaborate with nursing and therapy staff, document progress, and educate patients and families on therapy goals and outcomes. A TN license is required; sign-on bonus is offered.

Qualifications

• Graduated from an accredited Speech Language Pathology program.
• Valid TN professional license and registration.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Perform evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctual, professional, and adheres to scheduled hours.
• Performs duties assigned by Director of Rehabilitation.
• Complies with confidentiality, HIPPA and Residents' rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with specific goals.
• Set realistic goals based on patient capacity and tolerance.
• Document and record the patient's condition.
• Educate patients and families in an appropriate therapy method.
