**Speech Language Pathologist - PRN DAYS**

_Are you looking for a rewarding career in an award-winning healthcare system? We’re seeking a qualified Speech Language Pathologist to join our Texas Health family._

Work location: Texas Health Azle, 108 Denver Trail, Azle, Texas 76020

Work environment: Inpatient

Work hours: PRN DAYS (As Needed)

Here’s What You Need
  • Master's Degree in Speech Language Pathology or an equivalent post-baccalaureate degree recognized by the American Speech-Language-Hearing Association
  • CCC's and MBS experience required
  • Texas Speech Language Pathology License upon hire (SPL, SLP-I, or SLP preferred)
  • BCLS - Basic Cardiac Life Support certification prior to providing independent patient care, maintained quarterly
What You Will Do
  • Perform age-appropriate patient assessments and establish care plans
  • Obtain and verify physician orders for therapy and validate their appropriateness
  • Review medical records and obtain pertinent medical history from patients or families
  • Ensure informed consent is obtained from patients before treatment
  • Evaluate patients using appropriate assessment tools and measurements, including outcome assessment tools
  • Identify functional problems, prioritize care needs with patients and families, and set measurable, achievable goals
  • Determine care plans and therapeutic interventions to meet goals
  • Coordinate discharge planning with the care team and patients
  • Perform age-specific patient treatments using appropriate techniques and delegate procedures safely when necessary
  • Monitor patient responses and progress, and educate patients and families about treatment plans and safety issues
  • Maintain patient safety and communicate effectively with healthcare team members
  • Demonstrate professional accountability, utilize resources efficiently, and participate in training and mentorship
  • Attend departmental and community meetings, and meet productivity targets
  • Represent the department positively within the hospital and community
  • Complete all documentation accurately and timely, including assessments, treatment notes, discharge summaries, and regulatory compliance forms
  • Participate in continuous improvement activities and cross-training opportunities
