Responsibilities include:Develop individualized treatment plans and obtain appropriate physician authorization for services.Provide speech-language pathology services in accordance with physician orders and established treatment plans.Evaluate patients and develop appropriate goals and interventions based on clinical findings.Monitor and communicate patient progress, concerns, and changes in condition to supervisors, physicians, and interdisciplinary team members.Participate in Patient Care Conferences, Utilization Review Meetings, Rehabilitation Conferences, and other care coordination activities as needed.Complete evaluations, daily treatment notes, progress notes, recertifications, discharge documentation, and other required records accurately and timely.Document treatment changes and modifications in accordance with facility policies, professional standards, and payer requirements.Provide education and training to patients, families, caregivers, and nursing staff regarding therapeutic strategies, maintenance programs, and safe swallowing practices.Support effective discharge planning and transitions of care.Assist with staff education, in-service training, and clinical development programs.Maintain compliance with applicable Speech-Language Pathology and Audiology Board regulations, state practice acts, facility policies, and professional standards.Build positive, respectful relationships with patients, families, coworkers, and healthcare professionals.QualificationsCurrent and active Speech Language Pathologist (SLP) licenseStrong clinical, communication, and interpersonal skillsAbility to work effectively as part of an interdisciplinary healthcare teamExperience with patient assessment, treatment planning, and clinical documentationCompassionate, professional, and patient-centered approachStrong organizational skills and attention to detailMake a Difference With UsAt Shadelands Post Acute, we are committed to providing compassionate, high-quality care in a supportive environment.
