Job summary

Medical Hill Healthcare Center in Oakland is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis.   • Current State Speech-Language Pathologist (SLP) license.
  • CCC-SLP certificate preferred.
  • Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred.

  • Develop and implement individualized treatment plans based on physician orders and clinical findings.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Partner with physicians, nursing staff, families, and other healthcare professionals to coordinate care and optimize outcomes.
  • Complete timely and accurate documentation in accordance with facility policies and payer requirements.
  • Promote a safe treatment environment and support quality assurance initiatives.

Pay Rate: $60.00-$65.00 per hour

Join Our Rehabilitation Team

Medical Hill Healthcare Center is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our therapy team on a PRN basis. In this role, you will help residents improve their communication, cognitive-linguistic, and swallowing abilities while collaborating with an interdisciplinary team dedicated to delivering exceptional patient care.

Key Responsibilities
Resident Evaluation & Treatment
  • Assess residents to identify communication, cognitive, and swallowing deficits
  • Develop and implement individualized treatment plans based on physician orders and clinical findings
  • Monitor resident progress and modify treatment plans as appropriate
Interdisciplinary Collaboration
  • Partner with physicians, nursing staff, families, and other healthcare professionals to coordinate care and optimize outcomes
  • Participate in care planning meetings and discharge planning discussions
Education & Training
  • Educate residents and caregivers on treatment goals, home programs, communication strategies, and swallowing safety
  • Provide in-services and educational support to staff as needed
  • Mentor Clinical Fellows (CFY), students, and rehabilitation team members when applicable
Documentation & Compliance
  • Complete timely and accurate documentation in accordance with facility policies, regulatory standards, and payer requirements
  • Maintain compliance with all federal, state, and facility guidelines
Quality & Safety
  • Promote a safe treatment environment through adherence to infection control and safety procedures
  • Support quality assurance and performance improvement initiatives
Qualifications
  • Current State Speech-Language Pathologist (SLP) license
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience in skilled nursing, post-acute care, or geriatric rehabilitation preferred
  • Strong communication, organizational, and teamwork skills
