Speech Language Pathologist, SLP - PRN (Healthcare)

We are seeking a dedicated Speech Language Pathologist (SLP) to join our team at Hayward Gardens Post Acute.

Job Responsibilities:
  1. Develop effective treatment plans and obtain approval from the referring physician.
  2. Treat patients according to the physician’s treatment plan.
  3. Supervise Speech-Language Pathologist Assistants in patient care activities, following state practice acts.
  4. Communicate patient progress, issues, and plans with supervisors and healthcare team members.
  5. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  6. Conduct in-service training for staff members.
  7. Document evaluations, treatment notes, progress reports, and recertification documentation in compliance with state practice acts and payer requirements.
  8. Record treatment modifications as per policies and procedures.
  9. Educate patients’ families and nursing staff on maintenance programs and caregiver training for discharge readiness.
  10. Participate in discharge planning processes.
  11. Serve as a clinical preceptor for CFY/RFP Speech-Language Pathologists and student programs.
  12. Ensure compliance with the Speech Language Pathology and Audiology Bureau State Practice Act and facility policies.
  13. Maintain positive, effective, and appropriate relationships with patients, families, staff, and colleagues.
Qualifications:

Master’s or Doctorate degree in Speech-Language Therapy is required.
