About the Role

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Speech Language Pathologist (SLP) at Blue Point Healthcare Center. Our therapists are In-House Team Members. This is an exciting opportunity to make a direct impact with our residents through therapy. Qualifications

• Bachelors and/or a Master’s Degree in Speech-Language Pathology from an Accredited Program
• Valid or pending State Speech Language Pathology License

Responsibilities

• Evaluate, review, and screen patients’/residents’ medical charts, evaluations under physician order, and develop treatment plans based on patient needs
• Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and cognitive issues; administer, score, and interpret specialized speech tests
• Instruct residents, families, and caregivers on monitoring speech, practicing new skills, and diet modifications for safe swallowing
• Attend/Lead interdisciplinary meetings, relay clinical findings, and participate in family education
• Opportunity to treat at multiple locations to serve diverse resident needs
