Evolve Therapy Services, LLC seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Humboldt, TN. You will plan, direct, and provide comprehensive speech therapy services, ensuring adherence to CMS standards and patient-centered care.

The role emphasizes data-driven treatment planning, family and staff training, and coordination of FEES/MBS protocols, with opportunities for growth within the organization. This is a full-time on-site position in a facility setting.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure is expected.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocol implementation and site coordination.
• Participate in screening protocols to assess resident needs.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
