Evolve Therapy Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Tupper Lake, NY. This role supports quality resident care with data-driven treatment planning and family/staff coaching.

The position requires a Master’s in Speech Language Pathology, CCC, and an active SLP license. Multiple work formats are available, with responsibilities spanning evaluation, treatment planning, documentation, and program oversight.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC credential required.
• Active SLP license in practicing states.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate data for treatment planning and implementation.
• Develop a treatment plan with targets, short- and long-term goals.
• Instruct patient, family and staff in speech-language procedures for carryover.
• Complete CMS-aligned documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to identify needs timely.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
