Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an In-house Skilled Nursing Facility in Massena, NY. The role emphasizes planning, directing, and delivering speech-language pathology services to residents in accordance with federal, state, and local standards.

The candidate will evaluate residents, develop measurable treatment goals, and educate families and staff. Documentation will follow CMS expectations in NetHealth EMR, with oversight of FEES/MBS protocols.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC certification and active state license are required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning.
• Develop a treatment plan with target list and goals.
• Instruct patients, families, and staff for carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening protocols to ensure timely care.

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
