Job summary

Evolve Therapy Services in Selma, AL is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. The role focuses on planning, directing speech-language services and ensuring CMS-compliant documentation.

You will evaluate residents, develop treatment plans, and educate residents, families, and staff on communication strategies. Licensure in state and CCC are required, with ongoing CE.

Qualifications

• Must hold CCC and state licensure.
• Master's degree in Speech-Language Pathology is required.
• Continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services in line with standards.
• Evaluate and collect data for treatment planning.
• Develop treatment plans with goals and timelines.
• Instruct patients, families, and staff on speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate with site programs.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
