Job summary

Evolve Therapy Services seeks a licensed Speech Language Pathologist to join our in-house Skilled Nursing Facility team in Butler, AL on a PRN basis. You will provide patient-centered speech therapy services, develop plans, and coordinate with staff to support resident communication needs.

Responsibilities include conducting evaluations, creating short- and long-term goals, documenting in NetHealth, and ensuring CMS compliance.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• Evaluate and obtain data necessary for treatment planning and implementation
• Develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:
