Job summary

Evolve Therapy Services, LLC in Ohio seeks a Speech Language Pathologist for an in-house skilled nursing facility on a PRN basis. You will plan, direct, and deliver speech pathology services, ensuring high-quality resident care in compliance with federal and state standards.

You will evaluate patients, develop treatment plans with goals, educate families and staff, and ensure CMS-aligned documentation in NetHealth EMR. Licensure, CCC and ongoing CE are required.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice essential.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop and update treatment plans with goals and timelines.
• Educate patient, family, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident needs screening with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
