Evolve Therapy Services in Burlington, WI, is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time role (with a sign-on bonus) or PRN supports planning, developing, and directing speech-language pathology services to ensure high-quality resident care.

The ideal candidate holds a Master’s in Speech-Language Pathology, CCC-SLP, and an active state license, with ongoing professional development.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice required.
• Commitment to ongoing professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and department procedures.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patients, families, and staff in speech-language procedures to ensure carryover of therapeutic remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to assess residents' needs with clinical excellence.

Education

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time (Sign On Bonus) or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
