Evolve Therapy Services is a therapy management organization hiring compassionate therapists for our partner located in Crystal, MN.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility.

Job Type: Part Time or PRN.

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to ensure the highest quality of resident care.

Job Functions include:

  • Providing quality speech therapy services and initiating referrals when appropriate, following established standards of practice, therapy department procedures, and productivity standards.
  • Evaluating and gathering data necessary for treatment planning and implementation.
  • Developing a treatment plan with target lists, short-term, and long-term goals.
  • Instructing patients, families, and staff in speech-language procedures to facilitate carry-over of therapeutic goals in daily activities.
  • Completing compliant and medically necessary documentation in accordance with CMS expectations using NetHealth EMR systems.
  • Overseeing and implementing proper protocols for FEES/MBS in coordination with site programs.
  • Participating in screening protocols to meet residents' needs timely and with clinical excellence.

Education & Qualifications:

  • Master's degree in Speech Language Pathology.
  • Certificate of Clinical Competency.
  • Active SLP license in the state(s) of practice.
  • Ongoing continuing education and professional development to maintain licensure and certification.
