Empath Home Health, a member of Empath Health, is seeking a PRN Home Health SLP (Speech Language Pathologist). The PRN Home Health Speech Therapist provides treatment to the patient to improve sensory-motor development, perception, memory, language and cognition.

Locations: Home visits in the Fort Myers area/Lee county

Schedule: PRN

Responsibilities

• Provide speech therapy in compliance with physician’s prescription, patient’s plan of care, agency policies and procedures, and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments, to evaluate the client’s functioning, swallowing, and communication abilities.
• Develop a plan of care in consultation with the physician and care team.
• Instruct patients and families on proper food consistencies to maintain nutrition and prevent swallowing complications.
• Educate patients and families on strategies to improve sensory-motor development, perception, memory, language, and cognition.

Requirements

• Graduate from an accredited college program meeting educational requirements for a Certificate of Clinical Competence in speech pathology or audiology by the ASHA, or enrolled in a qualifying program with supervised experience in progress.
• State licensure as a speech pathologist.
• Preferred: One year of home health care experience.
