Job summary

VitalCaring Group is seeking a Hospice Speech-Language Pathologist to plan, organize, and deliver compassionate therapy focused on comfort, communication, and quality of life for patients in home health and hospice settings.

You will work with an interdisciplinary team to develop individualized care plans, educate families, and ensure documentation aligns with organizational policies. A supportive environment emphasizes growth, mentorship, and patient-centered care.

Qualifications

  • Master’s degree in Speech-Language Pathology with internship.
  • CCC from ASHA.
  • State licensure as Speech-Language Pathologist.
  • CPR certification current.
  • Valid driver’s license with insured vehicle.

Responsibilities

  • Conduct evaluations and assess home environment to determine equipment and speech/language needs.
  • Identify deficits and establish an individualized plan of care to improve or maintain communication function.
  • Administer speech therapy programs using specialized techniques and equipment.
  • Recommend communication devices or assistive aids as needed.
  • Instruct patients, families, caregivers, and agency personnel on treatment plans and strategies.
  • Collaborate with referring physicians and the interdisciplinary team for coordinated care.

Job Summary

Essential Functions
  • Conduct evaluations and assess the home environment to determine equipment and speech/language needs.
  • Identify functional speech and communication deficits; establish an individualized plan of care to improve or maintain essential function.
  • Administer speech therapy programs using specialized techniques and equipment.
  • Recommend communication devices or assistive aids as needed.
  • Instruct patients, families, caregivers, and agency personnel on treatment plans, communication strategies, and safe care techniques.
  • Collaborate closely with referring physicians and the interdisciplinary team to ensure comprehensive, coordinated care.
Communication & Documentation
  • Prepare and submit clinical notes in alignment with organizational policy.
  • Communicate ongoing patient needs, changes, and progress to physicians, caregivers, and team members.
  • Participate in interdisciplinary group (IDG) meetings, in-services, and other required team activities to support holistic care planning.
Additional Responsibilities
  • Maintain clinical competency through continued education and adherence to current SLP practices and theory.
  • Participate in peer consultation, orientation of hospice personnel, and quality performance improvement activities.
  • When appropriate, supervise hospice aides performing tasks related to speech-language therapy.
  • Perform other duties necessary to support effective delivery of SLP services.
Qualifications
  • Master’s degree in Speech-Language Pathology from an approved program, including required internship.
  • Meets education and experience requirements for the Certificate of Clinical Competence (CCC) from ASHA.
  • Certified and licensed to practice Speech-Language Pathology in the state of practice.
  • Current CPR certification.
  • Licensed driver with an insured, reliable automobile meeting organizational requirements.
  • Minimum of one year of clinical SLP experience.
  • Understanding of hospice philosophy and comfort-focused care.
