Job description

POSITION SUMMARY

The Speech-Language Pathologist contracted or employed through the agency is responsible for the implementation of standards of care for speech therapy services and for adherence to all conditions outlined in the Job Description or Independent Contractor Agreement.

ESSENTIAL RESPONSIBILITIES AND DUTIES
  • Conduct appropriate evaluations, assess home environment, and identify equipment needs relative to speech/language function as needed.
  • Assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, family, and caregiver.
  • Provide services ordered by the physician as indicated in the plan of care.
  • Provide patient, family, and caregiver counseling.
  • Provide patient, family, and caregiver education.
  • Prepare clinical notes.
  • Communicate with the physician responsible for the home health plan of care and other health care practitioners, as appropriate, related to the current home health plan of care.
  • Identify functional speech-language deficits and establish a plan of care to improve patient’s function.
  • Administer speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommend communication devices/aids as indicated.
  • Participate in required case conferences, in-services, and other meetings to ensure coordinated and comprehensive care.
  • Maintain clinical competency in speech language pathology practice and theory.
  • Identify patient, family, and caregiver needs for other home health services and refer as appropriate.
  • May supervise Home Health Aide in procedures directly related to the speech–language therapy plan of care.
  • Participate in agency’s quality assessment and performance improvement program and agency-sponsored in-service trainings.
  • Perform other appropriate services and special projects as assigned.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION
  • Current SLP licensure in state of practice.
  • If state licensure is not required, must have completed or be in the process of completing supervised clinical practicum of not less than nine (9) months full-time.
  • At least one (1) year of clinical experience working as a Speech-Language Pathologist in health care; home health experience preferred.
  • Current CPR certification required.
  • Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
PREFERRED KNOWLEDGE AND SKILLS
  • Excellent time management and problem‑solving skills.
  • Outstanding verbal and written communication skills.
SUPERVISORY RESPONSIBILITIES

May supervise Home Health Aide as needed.

Significant portion (>50%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation. Work schedule may occasionally require evening, night, holiday, or weekend assignments.
