Job summary

CareRite Centers is seeking a Speech Language Pathologist (SLP) to join the Gallatin Center for Rehabilitation and Healing team in Tennessee. This full-time role includes a sign-on bonus and highly competitive compensation.

The SLP will assess patients needing rehabilitation, develop and administer individualized treatment plans, and collaborate with a multidisciplinary team to ensure quality, patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid TN state license/registration.
• Experience with long-term/geriatric care preferred.
• Proficient in documentation writing.
• Experience with evaluations, progress notes, recerts, and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Punctual, professional, and adheres to schedule.

Responsibilities

• Assess referred patients requiring rehab services and provide direct patient care.
• Establish and administer a treatment program with goals per patient capacity.
• Set realistic goals and document patient condition and progress.
• Educate patients and families in appropriate therapy methods.
• Coordinate care with physicians and other disciplines to optimize outcomes.
• Maintain confidentiality, HIPAA, and Residents' Rights compliance.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
