Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team. The SLP will assess referred patients, provide direct care, and develop treatment plans under physician direction.

The role emphasizes goal setting, documentation, patient and family education, and collaboration with all disciplines to deliver patient-centered care. A TN license and accredited program graduation are required; sign-on bonus is offered.

Qualifications

  • Must have graduated from an accredited Speech-Language Pathology program.
  • Must have a valid TN license and registration.
  • Experience with long-term care or geriatric care preferred.
  • Proficient in documentation and progress notes.
  • Ability to collaborate with all disciplines for patient-centered care.

Responsibilities

  • Assess referred patients to determine rehab needs.
  • Provide direct patient care and administer treatment plans.
  • Set realistic goals and document patient condition and progress.
  • Educate patients and families on therapy approaches.
  • Collaborate with all disciplines to deliver care.
  • Maintain confidentiality and comply with HIPAA and Residents' Rights.
  • Perform additional duties as assigned by the Director of Rehabilitation.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:
  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of TN
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
