Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team in providing comprehensive rehabilitation services to residents.

The successful candidate will assess patients, develop and implement individualized treatment plans, document progress, and educate families while collaborating with interdisciplinary colleagues in a long-term care setting.

Qualifications

  • Graduate from an accredited Speech-Language Pathology program.
  • Active Tennessee license/registration.
  • Experience in long-term care or geriatric settings preferred.
  • Strong documentation and record-keeping skills.
  • Ability to perform evaluations, progress notes, recertifications, and discharge documents.
  • Collaborates with all disciplines to provide patient-centered care.
  • Demonstrates exceptional patient service and clinical excellence.
  • Maintain confidentiality and HIPAA compliance.

Responsibilities

  • Assess referred patients requiring rehab services and provide direct patient care.
  • Establish and administer treatment programs with measurable goals.
  • Document patient status and educate patients and families in therapy methods.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:
  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid professional license and registration for state of TN
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
  • Well versed with documentation writing
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence
  • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
