Job summary

Oasis Rehabilitation and Nursing in New York offers a full-time Speech-Language Pathologist position focused on patient assessments and direct care to determine rehabilitation potential. You will design and supervise treatment plans, set achievable goals, document patient progress, and educate families within a multidisciplinary team.

Requirements include an NY state license, CCCs, and experience in geriatric or long-term care settings.

Qualifications

• Must be licensed in NY and hold CCCs.
• Graduate from an accredited Speech-Language Pathology program.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills are required.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and Residents' Rights.

Responsibilities

• Assess patients and provide direct patient care to guide rehabilitation.
• Establish and administer treatment plans under physician direction.
• Set realistic goals, document progress, and educate patients and families.
• Collaborate with all disciplines to ensure comprehensive care.
• Complete evaluations, weekly progress notes, recertification and discharge documentation.
• Perform duties as directed by the Director of Rehabilitation.

Oasis Rehabilitation and Nursing has a Full-time opportunity for a Speech-Language Pathologist.

Candidates mustpossess a valid New York State License to practice Speech Language Pathology aswell as CCC’s (Certificate of Clinical Competence).

Our SLP is responsible for the assessment of patients: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech-Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.

As well, the Speech-Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for the state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide the best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by the Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Salary: $85,000.00-100,000.00 annually
