Evolve Therapy Services is a therapy management organization hiring compassionate Speech Language Pathologists (SLP) for an In-house Skilled Nursing Facility. This is a full-time opportunity with a split role at our partnered Burlington Health and Rehabilitation location in Burlington, WI, and Beloit Health and Rehabilitation in Beloit, WI.

Job Functions:
  1. Conduct regular screenings of residents to determine appropriate interventions or treatments based on scope of practice.
  2. Perform treatment plans for residents, following short- and long-term goals as established by supervising therapists.
  3. Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
  4. Establish individual resident therapy schedules in cooperation with team members.
  5. Conduct therapy according to the treatment plan, ensuring proper documentation and billing of services.
  6. Perform all other duties necessary to effectively treat patients.
  7. Ensure compliance with all CMS documentation standards and daily point of care documentation.
Education & Qualifications:
  • Master's degree in Speech Language Pathology.
  • Certificate of Clinical Competency.
  • Active SLP license in the state(s) of practice.
  • Ongoing continuing education to maintain licensure and certification.
