Job summary

Evolve Therapy Services, LLC is a therapy management organization seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Lamberton, MN. The role covers planning, directing, and providing speech pathology services in alignment with federal, state, and local standards.

The position supports multiple working arrangements: Full Time, Part Time, or PRN, with responsibilities including documentation, FEES/MBS oversight, and resident screening to ensure high-quality care.

Qualifications

• Must hold active SLP license in state(s) of practice.
• CCC credential required.
• Master's degree in Speech Language Pathology.
• Ability to complete CMS-compliant documentation.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident screening to identify needs and ensure timely care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility
Job Type: Full Time, Part Time, or PRN
Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.
Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
