Job summary

Evolve Therapy Services in Kentucky seeks a licensed Speech-Language Pathologist for an in-house Skilled Nursing Facility in Phelps. This part-time/PRN position focuses on delivering quality therapy, developing treatment plans, and ensuring CMS-compliant documentation.

Responsibilities include performing FEES/MBS oversight, data collection for progress, and training patients, families, and staff to carry over therapeutic goals in daily activities. Active state licensure required.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in states of practice.
• Ongoing continuing education and professional development.

Responsibilities

• Provide quality speech therapy services and initiate referrals as needed.
• Evaluate and obtain data for treatment planning.
• Develop treatment plans with targets and short-/long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures to generalize therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in screening to determine resident needs with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Part Time or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
