Job summary

Evolve Therapy Services in Waverly, NE seeks a part-time Speech Language Pathologist to provide in-house therapy at a Skilled Nursing Facility. You will plan, direct, and deliver services aligned with CMS standards and clinical best practices.

You will evaluate residents, develop treatment plans, and educate families and staff to ensure sustainable progress. Active licensure and CCC are required for practice in this role.

Qualifications

• Master's degree required in Speech-Language Pathology.
• CCC certification or equivalent credential required.
• Active state licensure in practice states is mandatory.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals (short/long term).
• Educate patient, family, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident needs screening with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Part Time

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
