Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to provide comprehensive speech therapy in an in-house skilled nursing facility in sunny South Florida. You will evaluate residents, develop treatment plans, and coordinate care with families and staff.

The role offers a 6-month contract-to-permanent path with a flexible schedule and relocation assistance. Full-time, part-time, or PRN options are available, with CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency (CCC-SLP)
• Active SLP license in state(s) of practice
• Ongoing continuing education and licensure maintenance

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate
• Evaluate and obtain data for treatment planning and implementation
• Develop treatment plans with target lists and goals
• Instruct patient, family, and staff in speech-language procedures for carry-over
• Complete CMS-compliant documentation in NetHealth EMR
• Oversee FEES/MBS protocols and coordinate site programs
• Participate in screening protocols to ensure timely resident needs

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type

Full Time (Flexible Schedule + Relocation Assistance!), Part Time, or PRN

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
