Evolve Therapy Services, LLC in sunny Florida seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. This is a 6-month contract-to-permanent opportunity with flexible schedule and relocation assistance, plus options for Part Time or PRN.

You will plan, organize, and direct Speech Language Pathology services, ensuring compliance with federal, state, and CMS guidelines while delivering high-quality resident care.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice and department procedures
• Evaluate and obtain data necessary for treatment planning and implementation
• Develop treatment plans with target lists, short-term and long-term goals
• Instruct patient, family, and staff in speech-language procedures for carryover in daily activities
• Complete compliant and medically necessary documentation per CMS expectations in NetHealth EMR
• Oversee and implement FEES/MBS protocols and coordinate site programs
• Participate in screening protocols for establishing resident needs in a timely and clinically excellent manner

Education

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• MUST have active SLP license in state(s) of practice.
