Yorktown Manor has an exciting opportunity for a 20-hour per week PRN Speech Language Pathologist to join their In-House Rehab team in Yorktown, IN.

Yorktown Manor is an outstanding skilled nursing facility offering residents in Delaware County, Indiana, a unique and energetic approach to rehab. We provide short-term and long-term rehab, memory care, and hospice services. Our focus is on premier care, a warm home-like environment, and engaging activities that support recovery.

Our mission is to transform rehabilitation and nursing care in the Yorktown community, aiming to be the top provider of post-acute healthcare for both short-term and long-term patients.

We are a comprehensive skilled nursing facility near area hospitals, specializing in post-acute care tailored to individual needs, including physical, occupational, and speech therapy, pharmacogenetics, and staff training.

Salary Range: $50 - $60 per hour

Essential Duties and Responsibilities
  1. Evaluate residents' conditions related to swallowing or cognitive deficits.
  2. Assist residents in achieving goals to improve cognition and swallowing.
  3. Provide education to residents and caregivers.
  4. Conduct skilled speech and language assessments and develop individualized treatment plans.
  5. Implement augmentative and alternative communication (AAC) systems.
  6. Train and educate clients, families, caregivers, and staff as a subject matter expert.
  7. Coordinate with other professionals for diagnostic swallowing procedures as ordered.
  8. Maintain competent technical skills according to current standards.
  9. Communicate changes in residents' conditions promptly.
  10. Accurately document clinical assessments and billing records.
  11. Respond timely to family requests and concerns.
  12. Collaborate with restorative nursing and care staff for training.
  13. Ensure resident safety, preventing abuse or neglect.
  14. Maintain confidentiality and comply with legal and professional standards.
