Job summary

Lutheran Sunset Ministries in Clifton, TX is hiring a Speech-Language Pathologist (SLP) to join our rehabilitation team. This full- or part-time position offers an opportunity to make a meaningful difference in care for residents requiring speech, language, cognitive-communication, voice and swallowing therapies.

The SLP will evaluate, diagnose, and treat residents, collaborate with an interdisciplinary team, ensure documentation compliance, and participate in care planning conferences.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Texas licensure as a Speech-Language Pathologist.
• CCC-SLP preferred; CFY considered.
• SNF/long-term care experience preferred.
• Knowledge of current speech-language pathology treatment practices.
• Experience with EMR documentation systems preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings.
• Provide skilled speech-language pathology services, including treatment for communication, cognitive, and swallowing disorders.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers regarding treatment goals and strategies.
• Maintain accurate and timely documentation in compliance with company policies and state and federal regulations.
• Collaborate with physicians, nursing staff, rehabilitation team members, and other healthcare professionals to coordinate care.
• Participate in interdisciplinary meetings and care planning conferences.
• Ensure compliance with all professional standards and regulatory requirements.
