Description

Paradigm at the Oaks has an exciting opportunity for a In-House, Consistent PRN Speech Language Pathologist in Schulenburg, TX.

Job Summary

The Speech Language Pathologist (SLP) will diagnose, plan, and treat speech, language, swallowing, and cognitive disorders. The SLP will also document patient treatments, changes in condition, and other relevant information.

Responsibilities
  • Evaluate residents for swallowing or cognitive deficits.
  • Assist residents in improving cognition or swallowing abilities.
  • Educate residents and caregivers.
  • Provide skilled, medically necessary speech and language assessments and develop individualized treatment plans to enhance independence and functioning.
  • Implement augmentative and alternative communication (AAC) systems for individuals with severe expressive or comprehension disorders.
  • Train and educate clients, families, caregivers, and professionals as a subject matter expert.
  • Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by a physician.
  • Maintain competent technical skills according to current standards of practice.
  • Communicate any changes in residents' conditions.
  • Accurately complete billing records and clinical documentation.
  • Respond promptly to family requests and concerns.
  • Coordinate and train with Restorative Nursing and other care staff.
  • Ensure resident safety by preventing abuse or neglect.
  • Maintain confidentiality and safeguard patient information.
  • Comply with all legal and professional requirements at federal, state, and local levels.

Qualifications
SLP Skills
  • Preferred experience in SNF/Long-term care settings.
  • Knowledge of current treatment practices.
  • Familiarity with Net Health Electronic Documentation System is a plus.
  • Ability to work with residents of diverse personalities.
Required Licensure
  • Active and valid Texas license in Speech-Language Pathology.
  • Current ASHA Certification.
  • In good standing with all regulatory agencies and licensing boards.
