Job summary

Trilogy Health Services in Indiana is seeking a Speech-Language Pathologist (SLP) to provide comprehensive language, voice, and swallowing therapies in an interdisciplinary setting. You will assess patients, design individualized therapy plans, and document progress in line with regulatory standards.

The position requires state SLP licensure and a CCC-SLP is preferred or required. CPR certification is preferred; travel is expected.

Qualifications

• Master's degree in Speech-Language Pathology is required.
• Current, valid state SLP licensure is required.
• CCC-SLP from ASHA is preferred or required.
• Current CPR certification is preferred.

Responsibilities

• Evaluates patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develops and implements individualized therapy plans based on assessment findings, setting measurable goals.
• Provides therapy to improve communication and swallowing, including articulation, language, voice, and fluency.
• Educates patients and families about disorders, therapy techniques, and strategies to support progress.
• Maintains accurate documentation of evaluations, progress notes, and treatment plans in accordance with policies and regulations.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.

Qualifications

The Speech Language Pathologist (SLP), is responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Evaluates patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develops and implements individualized therapy plans based on assessment findings, setting measurable goals to address patients' specific needs.
• Provides therapy to improve patients’ communication skills, including articulation, language comprehension, voice modulation, and fluency, as well as swallowing and feeding abilities.
• Educates patients and their families about the nature of disorders, therapy techniques, and strategies to support progress and enhance communication and swallowing in daily life.
• Maintains accurate and up-to‑date documentation of patient evaluations, progress notes, and treatment plans in compliance with facility policies and regulatory requirements.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master DegreeExperience: 0-1 years

Licenses and Certifications:

Must have and maintain a current, valid state SLP licensure

Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred or required

Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
