We are looking for a dedicated SLP to join our rehabilitation team of outstanding professionals who provide person-centered care to our residents and patients. Our goal is to help each individual achieve meaningful outcomes while maintaining the highest standards of quality and compassion.

Competitive Pay: $42-$55 per hour

Evaluate residents for speech, language, cognitive, communication, and swallowing deficits.
Develop and implement individualized treatment plans to improve cognition, communication, swallowing, and overall functional independence.
Provide skilled, medically necessary speech-language pathology assessments and therapeutic interventions in accordance with established plans of care.
Support residents in achieving their individualized cognitive, communication, and swallowing goals.
Provide education, training, and guidance to residents, families, caregivers, and other healthcare professionals.
Coordinate with physicians and other healthcare professionals for diagnostic swallowing procedures, including Modified Barium Swallow Studies (MBSS) and Fiberoptic Endoscopic Evaluation of Swallowing (FEES), as ordered and clinically indicated.
Apply current clinical standards and evidence-based practices in speech-language pathology.
Monitor resident progress and communicate changes in condition to the appropriate members of the healthcare team.
Maintain accurate and timely clinical documentation and billing records in accordance with facility policies and applicable regulations.
Respond to family questions, requests, and concerns in a timely and professional manner.
Coordinate with Restorative Nursing and other care staff to support residents’ ongoing functional needs.
Maintain confidentiality and safeguard all resident and patient information in accordance with applicable privacy requirements.
Comply with all federal, state, and local legal, regulatory, and professional requirements.
Attend required meetings and perform other duties as assigned by the Director of Rehabilitation.
Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.
Knowledge of speech-language pathology treatment practices, techniques, and interventions.
Familiarity with the Net Health electronic documentation system preferred.
Strong clinical, communication, and interpersonal skills.
Ability to work effectively with residents and patients with a variety of needs, personalities, and abilities.
Strong organizational skills and attention to detail.
Ability to collaborate effectively with residents, families, caregivers, and interdisciplinary team members.
Commitment to compassionate, person-centered care.
Active and valid Speech-Language Pathology license in Vermont.
Current ASHA certification.
Must be in good standing with all applicable regulatory agencies and licensing boards.
