Job summary

Elara Caring is seeking a compassionate PRN Medical Social Worker to support home health patients and families in Portland and southern Maine. You will work independently in patients' homes while collaborating with an interdisciplinary team to ensure timely access to resources and services.

The role requires an MSW or higher, a current Maine license, and at least 1 year of healthcare social work experience. Travel up to 50% and strong documentation skills are essential.

Qualifications

• MSW or Doctoral degree in Social Work from CSWE-accredited program.
• Maine Social Worker license in good standing.
• At least 1 year of social work experience in healthcare.
• Willingness to travel 50% of the time.
• Ability to lift up to 50 pounds and transfer patients when needed.

Responsibilities

• Assess psychosocial, emotional, financial, and environmental needs.
• Complete initial social work evaluations and follow-up visits.
• Provide counseling, education, advocacy, and emotional support.
• Connect patients and caregivers with community programs and resources.
• Identify barriers affecting safety, recovery, or independence.
