Job summary

Elara Caring is seeking a PRN Medical Social Worker (MSW) to support home health patients and families across Portland and southern Maine. You’ll work independently in the community, advocating for patients and helping families access community resources and plans of care.

Requirements include a Master’s or Doctoral degree in Social Work, Maine licensure, and at least 1 year in healthcare social work, with 50% travel expected and a valid driver’s license.

Qualifications

• Master's or Doctoral degree in Social Work from a CSWE-accredited program.
• Current Maine Social Worker license in good standing.
• At least one year of social work experience in a healthcare setting.
• Strong communication, counseling, documentation, and advocacy skills.
• Ability to travel throughout the assigned service area with 50% travel required.
• Must possess a valid driver's license or reliable transportation.

Responsibilities

• Assess patients' psychosocial, financial, and environmental needs.
• Complete initial social work evaluations and follow-up visits.
• Provide counseling, education, and emotional support to patients and caregivers.
• Connect patients and families with community programs and financial resources.
• Identify barriers that may affect a patient's safety or ability to remain at home.
• Collaborate with physicians, nurses, therapists, and other care team members.
• Support discharge planning and continuity of care.
• Complete accurate and timely clinical documentation.

A safe recovery at home often depends on more than medical care. Patients may also need emotional support, financial resources, transportation, housing assistance, or help navigating community programs.

Elara Caring is hiring a PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities.

You will work independently in the community while receiving support from an experienced interdisciplinary team. Every visit gives you an opportunity to remove barriers, advocate for your patients, and help families move forward with confidence.

What You’ll Do

• Communicate changes or concerns affecting patient safety and well-being
• Complete accurate and timely clinical documentation

What You’ll Bring

• Master's or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education
• Current Maine Social Worker license in good standing
• At least one year of social work experience in a healthcare setting
• Strong communication, counseling, documentation, and advocacy skills
• Ability to work independently in patients' homes
• Willingness to travel throughout the assigned service area with 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
