Join Our Team as a Speech Language Pathologist (SLP) at Creative Health Solutions

Creative Health Solutions is seeking a PRN Speech Therapist to join our growing team. We are a therapist-operated company providing therapy services at Skilled Nursing facilities since 2001. Our mission is to develop great clinicians, provide cutting-edge rehab in a caring environment, and support autonomy that respects our therapists' clinical decision-making skills. We pride ourselves on our clinical excellence and our commitment to high-quality patient care.

Responsibilities:

• Review patient information and evaluate its impact on assessment and treatment.
• Conduct and document functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Revise treatment plans as needed during therapy.
• Complete documentation in compliance with legal and clinical standards.
• Report to work as scheduled.

Minimum Qualifications:

• Master of Science in Speech/Language Pathology.
• Current license/certification in the practice state.
• Meeting educational requirements for certification or in the process of obtaining the Certificate of Clinical Competence from ASHA.
• Competency in applying speech therapy principles, methods, and equipment.
• Excellent communication and interpersonal skills.
• Computer skills for documentation and job duties.
