Trilogy Health Services is seeking a Speech-Language Pathologist to provide comprehensive evaluation, therapy planning, and intervention in a senior living setting. The role includes coordinating with an interdisciplinary team to improve communication and swallowing, documenting progress, and educating families.

The candidate should hold a Master’s degree in Speech-Language Pathology with state licensure; CCC-SLP is preferred or required, and CPR is beneficial. Travel is minimal.

Qualifications

• Evaluates patients to identify speech, language, voice, and swallowing disorders through assessments and interviews.
• Develops and implements individualized therapy plans with measurable goals.
• Provides therapy to improve communication, voice, fluency, and swallowing as needed.
• Educates patients and families about disorders, therapy techniques, and progress strategies.
• Maintains accurate documentation of evaluations, progress notes, and plans.

Qualifications

The Speech Language Pathologist (SLP), is responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

• Evaluates patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develops and implements individualized therapy plans based on assessment findings, setting measurable goals to address patients' specific needs.
• Provides therapy to improve patients’ communication skills, including articulation, language comprehension, voice modulation, and fluency, as well as swallowing and feeding abilities.
• Educates patients and their families about the nature of disorders, therapy techniques, and strategies to support progress and enhance communication and swallowing in daily life.
• Maintains accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans in compliance with facility policies and regulatory requirements.
• Collaborates with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master DegreeExperience: 0-1 years

Licenses and Certifications:

Must have and maintain a current, valid state SLP licensure

Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred or required

Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
